Sore throat or strep throat? How to tell
A sore throat is a symptom, not a diagnosis. Four times out of five it is viral, brought on by a cold, the flu or another common bug, and antibiotics do nothing for it. Strep throat is different: it is a bacterial infection, it tends to arrive suddenly and hit harder, and it is the type that sometimes warrants antibiotics.
There is no single symptom that proves it either way, but the pattern matters. Strep is more likely when the throat pain comes on fast, there is a fever, the tonsils are red or have white patches, the glands in the neck are swollen, and there is no cough or runny nose. The more of those you have, the more likely it is bacterial, which is exactly what our prescriber weighs up in the consultation.

Do you actually need antibiotics for a sore throat?
Often, no. This is the part some services skate over, so we will be straight with you: most sore throats, including many that turn out to be strep, get better on their own, and antibiotics only shave a day or so off the worst symptoms. Taking them when they are not needed does nothing for you and adds to antibiotic resistance, which is why responsible prescribing means saying no sometimes.
Antibiotics earn their place when the picture points strongly to a bacterial infection, when symptoms are severe or dragging on, when there is a higher-than-usual risk of complications, or in young children where strep is more common and more consequential. What treatment for strep throat looks like
Where antibiotics are appropriate, the usual first choice for strep throat is a penicillin (phenoxymethylpenicillin), taken as a short course, typically around ten days. For anyone allergic to penicillin, an alternative such as clarithromycin is used instead. Your prescriber will confirm the right choice, dose and length for you and go through how to take it and what to expect.
Alongside any antibiotic, simple self-care does most of the heavy lifting: paracetamol or ibuprofen for pain and fever, plenty of fluids, rest, and soothers like lozenges or a warm honey drink. Finish the full course of any antibiotic even once you feel better.
Strep throat is contagious until you have been on antibiotics for about 24 hours, or for longer if you are not taking them, so stay off work or school and avoid close contact until then.
