Mounjaro has become one of the most talked-about weight loss treatments in the UK, and for good reason. In clinical trials, patients taking Mounjaro lost up to 22.5% of their body weight over 72 weeks on the highest dose. That makes it one of the most effective weight loss medications currently available.
But Mounjaro is not a miracle cure. It works best alongside a healthy diet, regular movement, and consistent effort. Think of it as a powerful support system that helps you lose weight, but it cannot do the work alone. When used correctly, Mounjaro can help reset your relationship with food and give you the momentum needed to achieve lasting results.
In this guide, we’ll walk you through what Mounjaro results to expect, week by week – from your very first dose through to long-term maintenance. You’ll learn how your body may respond at each stage, what side effects to prepare for, how much weight you can realistically expect to lose, and how to get the best possible results from your treatment.
Whether you’ve just started Mounjaro or you’re researching what to expect before committing, this guide will give you a clear, honest picture of the journey ahead, backed by clinical evidence from the SURMOUNT-1 trial and our own experience supporting patients at Longridge Travel Health Clinic.

How Does Mounjaro Work?
Mounjaro contains the active ingredient tirzepatide, which is a dual-action GIP and GLP-1 receptor agonist. In simpler terms, it mimics two natural hormones that your body already produces to regulate appetite and energy use.
One hormone (GLP-1) helps you feel full for longer and slows down how quickly your stomach empties. The other (GIP) works alongside GLP-1 to improve how your body manages energy and processes food. Together, they reduce hunger, lower cravings, and help you eat less without feeling deprived.
This dual-action approach is what sets Mounjaro apart from single-action GLP-1 treatments like Wegovy (semaglutide). In a head-to-head study, tirzepatide produced greater average weight loss than semaglutide over the same period.
Everyone starts Mounjaro at 2.5mg per week. The dose is increased gradually every four weeks, up to a maximum of 15mg. This stepped approach helps your body adjust and reduces the chance of side effects.
Mounjaro Dosage Schedule
Below is the standard dosage escalation plan for Mounjaro. Your clinician may adjust this timeline based on how your body responds and how well you tolerate each dose.
| Weeks | Dose | Purpose |
|---|---|---|
| 1–4 | 2.5mg | Starting dose – allows your body to adjust. Not a therapeutic dose for weight loss. |
| 5–8 | 5mg | First maintenance dose -appetite suppression becomes more noticeable. |
| 9–12 | 7.5mg | Continued titration -stronger effects on appetite and weight. |
| 13–16 | 10mg | Advanced dose – significant appetite control for most patients. |
| 17–20 | 12.5mg | Approaching maximum – only if tolerated and needed. |
| 21+ | 15mg | Maximum maintenance dose – used for the remainder of treatment. |
Note: You do not have to increase your dose every four weeks. Your clinician will work with you to decide the right time to move up based on your progress and side effects. It’s perfectly fine to stay on a lower dose for longer if it’s working well for you.
What to Expect with Your First Dose of Mounjaro
Taking your first Mounjaro injection can feel daunting, but knowing what to expect makes the process much easier.
Before your injection: Store your Mounjaro pen in the fridge (between 2°C and 8°C). Take it out around 30 minutes before injecting to allow it to reach room temperature. Choose a day and time that you can stick with each week as consistency will help maintain steady medication levels in your body.
Where to inject Mounjaro: You can inject Mounjaro into your abdomen, thigh, or upper arm. It is recommended to rotate the site each week to reduce irritation. The injection itself takes only a few seconds and most people describe it as a small pinch.

The first 24-72 hours after Mounjaro: Many people begin to notice subtle appetite changes within 24 to 48 hours of their first dose. You may find that you feel full sooner during meals or that your usual cravings feel less intense. Some people feel very little difference on day one and this is completely normal. The medication is beginning to work in the background, even if you can’t feel it yet.
Early Mounjaro effects to look out for: : If side effects occur, they typically peak between 24 and 72 hours after injection. The most common side effects are mild nausea, bloating, and changes in bowel habits. These will usually settle within a few days. Eating smaller, blander meals and staying well hydrated can help reduce or manage side effects.
Weight loss on your first dose: Do not expect the scales to change significantly after one injection. Any early drop is likely to be water weight, not fat loss. True weight loss builds gradually over the coming weeks as the medication reaches a steady level in your body.
Mounjaro Week 1: Results, Side Effects & What to Expect
Your first full week on Mounjaro is an adjustment period. You’re on the lowest dose (2.5mg), which is designed to help your body get used to the medication rather than produce dramatic weight loss.
What you may notice
Some people notice reduced appetite within the first few days. You might find yourself leaving food on your plate, snacking less, or simply not thinking about food as often. Others feel very little difference during week one – both responses are completely normal.
Common side effects in Week 1
The most commonly reported side effects during the first week include mild nausea, reduced appetite, constipation, diarrhoea, stomach cramps, bloating, and headaches. These are typically mild to moderate and are a sign that your body is adjusting to the medication. We found that nausea was the most common side effect, affecting around 24–31% of participants depending on their eventual dose, with most cases being mild or moderate.
If side effects feel too strong, you can speak to your clinician about staying on the starting dose for longer before increasing. This gives your body more time to adapt.
Tips for your first week of Mounjaro
When you’re just starting out, try to eat smaller meals more frequently rather than three large ones. Stay well hydrated and aim for at least 2 litres of water per day. Avoid rich, fatty, or greasy foods, especially around injection day. Starting gentle movement like daily walks of 1,000–2,000 steps if you’re not currently active will also help. We recommend keeping a journal to track how you feel, what you eat, and any side effects.
Mounjaro Week 2: What to Expect on Your Second Dose
Week 2 on Mounjaro often feels very similar to week 1, as you’re still on the same 2.5mg dose once a week for the first four weeks. This gives your body time to adjust gradually while the medication is beginning to build a more consistent presence in your system.
Some people may begin to notice stronger appetite suppression during their second week. You may find it easier to resist cravings, feel satisfied with smaller portions, or naturally gravitate towards lighter meals. For others, week 2 is when early side effects begin to ease, especially if they were mild to begin with.
Second dose tip: Take your second injection on the same day of the week as your first. You don’t need to inject at the exact same time, but keeping to a consistent schedule helps maintain steady medication levels and makes it easier to build a routine.
It’s important to remember that Mounjaro is not a quick fix. To get the most from it, you need to combine it with a health and balanced, calorie-controlled diet and regular movement. Even small changes at this stage like adding a short walk after meals or swapping a processed snack for a piece of fruit will support long-term weight loss and help you feel your best throughout treatment, laying the groundwork for long-term success.
Mounjaro Weeks 3 & 4 – What to Expect After Your First Month
By weeks 3 and 4, most people find that any initial side effects have eased significantly. You’re still on the 2.5mg starting dose, meaning your body should now be well adjusted to the medication and the weekly injection routine.
This is often when Mounjaro’s effects start to become more noticeable. Many people report feeling fuller for longer, having fewer food cravings, and finding it easier to stick to smaller portions. Clinical trial data from SURMOUNT-1 shows that some patients lose up to 4% of their body weight in the first month alone. For someone weighing 100kg, that’s around 4kg in the first month.
At this stage, the tirzepatide in your body is reaching what’s known as a “steady state” meaning levels remain more consistent throughout the week rather than spiking and falling. This means the appetite-suppressing effects should feel more sustained.
If you haven’t noticed much weight loss yet, don’t be discouraged. Everyone responds differently, and the 2.5mg dose is primarily about adjustment, not maximum results. The real progress typically begins when your dose increases in the coming weeks.
While everyone’s experience is different, the effects of Mounjaro should become more noticeable around this time. Many people feel fuller for longer and start seeing the benefits of reduced appetite and fewer cravings.
Mounjaro First Month Results Summary
- Weight loss of up to 4% of starting body weight
- Side effects usually settle down by week 3–4
- Noticeable appetite reduction for most people
- The dose should remain at 2.5mg throughout

Mounjaro Weeks 5–8: Moving to 5mg
At week 5, your Mounjaro dose will typically increase from 2.5mg to 5mg. This is the first therapeutic maintenance dose, and for many people it’s when results start to become more visible. You’ll stay on 5mg for four weeks before any further increase.
It’s important to remember that this is just a general timeline/guide. Your journey may look different depending on how your body responds. At Longridge Travel Health Clinic, we conduct regular check-ins to review your progress and decide together whether it’s time to increase your dose or stay where you are. Everything is tailored to your individual needs.
💡 All of our patients receive ongoing monitoring and tailored dose reviews as part of their treatment plan. We offer nationwide delivery across the UK.

Many people lose around 5 to 6% of their body weight during weeks 5–8. For someone who started at 90kg, that could mean a total loss of around 4.5–5.5kg by this point. Clothes may start to fit differently, and you might begin to notice changes in the mirror. If your results are different, that’s completely normal. Progress varies, and the number on the scale doesn’t tell the full story.
Dose increase and side effects: Because the dose has increased some side effects from earlier weeks may briefly return. Nausea, constipation, or changes in bowel habits are usually the most common. This is completely normal and usually settles within a few days to a week. If symptoms are difficult to manage, speak to your healthcare provider – they can advise and help you decide on whether to continue, pause, or adjust your plan.
Appetite changes in Month 2: At the 5mg dose, most people will notice a significantly stronger reduction in appetite. You’ll likely feel full sooner and experience fewer cravings. This makes it much easier to maintain a calorie deficit without feeling like you’re constantly fighting hunger.
Weighing Yourself: Whilst you are on Mounjaro, try to avoid weighing yourself every day as it can be mentally exhausting and won’t reflect your true fat loss. Your weight naturally goes up and down from water retention, hormones, and digestion. We would recommend weighing yourself every one to two weeks instead as it gives a much more accurate picture of your overall progress and is much better for your mental well-being. Try to weigh yourself at roughly the same time of the day before any meals and after emptying your bowels to track your progress more accurately.
Keeping up healthy habits that you started in the earlier months like a balanced diet and regular exercise gives Mounjaro the best chance to work effectively, and also lays the foundation for long-term success. The more consistent you are now, the easier it becomes to maintain your results after treatment ends
Mounjaro Weeks 9 to 12: Progressing to 7.5mg
At week 9 your dose increases again, this time from 5mg of tirzepatide to 7.5mg. You’ll remain on this dose for the next four weeks. By this stage, you will be on month 3 of your Mounjaro journey. Clinical studies suggest that participants have lost around 8–9% of their total body weight on average.
As with previous dose increases, you may experience a brief return of side effects such as nausea, constipation, or headaches. This is your body adjusting to the higher level of medication and usually passes within a few days.
Managing Side Effects at Higher Mounjaro Doses
We’ve included some tips below on what you can do to help manage the side effects:
- Eat smaller meals more frequently throughout the day.
- Drink plenty of water as being dehydrated can make nausea and constipation worse.
- Get enough sleep and rest to support your body’s adjustment.
- Use over-the-counter remedies for constipation or diarrhoea if needed.
- Keep moving with gentle exercise; even a 20-minute walk can help with digestion and energy levels.
If side effects become unmanageable or symptoms worsen, contact your healthcare provider or reach out to us at Longridge Travel Health Clinic. You should never feel like you have to push through alone, and sometimes a small adjustment to timing or dose is all that’s needed.
By weeks 9–12, many people find that weight loss feels more consistent and sustainable. The combination of strong appetite suppression and healthy habits you’ve been building creates real momentum and results. This is often the point where most of our patients say they feel confident that the treatment is genuinely working.
Mounjaro Week 13 and Beyond: Reaching Your Maintenance Dose
From week 13, your Mounjaro dose will increase to 10mg. Over the following months, your dosage will continue to rise in 2.5mg increments every four weeks until you reach the maximum maintenance dose of 15mg by around week 21. Once you reach your 15mg maintenance dose, you’ll stay on it for the remainder of your treatment.
Total treatment duration varies based on your individual needs, goals, and how your body responds. We work closely with every patient to ensure their journey is tailored and effective.
Appetite at higher doses: As your dose increases, appetite suppression becomes notably stronger. You’ll still feel hungry at times, but it should be much easier to reduce your calorie intake and stick to smaller portion sizes. Many people find that they naturally stop snacking as often or begin to lose interest in sugary foods.
If you’re still finding yourself snacking frequently or reaching for sugary foods, it may be habit rather than true hunger. This is incredibly common and nothing to feel bad about. Reach out to us and we can help you with strategies to break those patterns especially if stress or emotional eating is a factor. Support is available, and you don’t have to break those habits on your own.
At the 15mg maximum dose over 72 weeks, the SURMOUNT-1 clinical trial data reported an average weight loss of 22.5% of starting body weight. At 10mg, the average was 21.4%, and at 5mg it was 16%. Crucially, over 90% of participants on the higher doses lost at least 5% of their body weight, which is the threshold considered clinically meaningful for metabolic health improvements.

How Quickly Does Mounjaro Work?
This is one of the most common questions we hear from patients. The short answer is: Mounjaro starts working in your body from the very first dose, but visible results take time to build.
- Within 24–48 hours: Many people notice subtle changes in appetite. You may feel full sooner during meals or find that cravings feel less urgent.
- Within 1–2 weeks: Appetite suppression typically becomes more consistent. Some people see a small drop on the scales, though this is often water weight rather than fat loss.
By week 4: The medication reaches a steady state in your body. Clinical trials show an average weight loss of around 4% by this point. - Weeks 5–12: As the dose increases, weight loss accelerates. Most people find this is when results become clearly visible.
- Months 3–6+: Weight loss continues steadily, though the rate may slow compared to the early months. The full effects of Mounjaro are typically seen after 72 weeks of treatment.
If you’re not seeing results as quickly as you expected, don’t give up. Research from the clinical trials show that 90% of “late responders” (those who lost less than 5% in the first 12 weeks) still went on to achieve clinically meaningful weight loss by week 72. The key is to stay consistent with both the medication and your healthy habits.
Can You Stay on 2.5mg Mounjaro?
This is another question we get asked regularly, and the answer is a little complicated.
The 2.5mg dose is officially classed as a starting dose, not a therapeutic maintenance dose. It’s designed to help your body adjust to the medication and reduce the likelihood of side effects. In clinical trials, the lowest maintenance dose studied was 5mg.
That said, some people do experience appetite suppression and early weight loss at 2.5mg. If you’re seeing results and tolerating the medication well, your clinician may decide to keep you on 2.5mg for longer before increasing. There’s no strict requirement to increase every four weeks – the timeline we’ve discussed should be guided by your individual response.
However, for most people, the significant and sustained weight loss seen in clinical trials is associated with the higher doses (5mg, 10mg, and 15mg). If your goal is meaningful, long-term weight loss, gradually increasing the dose as tolerated is generally recommended.
The best approach is to work closely with your clinician, who can monitor your progress and advise on the right dose for you.
Do You Have to Increase Your Mounjaro Dose?
No, you do not have to follow the standard four-week dose increase schedule. While the escalation plan is designed for most patients, everyone’s body responds differently.
Some people do well on lower doses and don’t need to reach the maximum 15mg. Others may need to stay on a particular dose for longer than four weeks to allow side effects to settle before moving up. Both approaches are perfectly valid.
What matters most is that any dose changes are made in consultation with a healthcare professional who can assess your progress, manage side effects, and ensure the treatment is working safely and effectively for you.
Reasons your clinician might suggest staying on a lower dose include: you’re already achieving good weight loss results, side effects are significant and need more time to resolve, or you have other health considerations that make a slower approach safer.
How Much Weight Can You Lose on Mounjaro?
The amount of weight you lose on Mounjaro depends on several factors: your starting weight, the dose you reach, how closely you follow a healthy diet and exercise plan, and how your individual body responds to the medication.
The best evidence comes from the SURMOUNT-1 clinical trial, which studied 2,539 adults with obesity or overweight over 72 weeks. The results by maintenance dose were as follows:
- 5mg/week Maintenance Dose: Resulted in an average weight loss of 16.0% with over 85% of participants losing more than 5% of their body weight and 32% losing over 20%
- 10mg/week Maintenance Dose: Resulted in an average weight loss of 21.4% with over 89% of participants losing more than 5% of their body weight and 50% losing over 20%
- 15mg/week Maintenance Dose: Resulted in an average weight loss of 22.5% with over 91% of participants losing more than 5% of their body weight and 57% losing over 20%
- Placebo: Resulted in an average weight loss of 2.4% with 35% of participants losing more than 5% of their body weight and only 3% losing over 20%
Source: Jastreboff et al., SURMOUNT-1, NEJM 2022. Efficacy estimand results at 72 weeks.
These are impressive figures, but remember: Mounjaro works best as part of a comprehensive approach that includes dietary changes and increased physical activity. The patients who achieve the best long-term results are those who use the medication as a tool to build sustainable healthy habits not as a replacement for them.
Frequently Asked Questions About Mounjaro
How long does Mounjaro take to start working?
Mounjaro begins acting in your body from the first dose. Most people notice appetite changes within 24 to 72 hours. Measurable weight loss typically starts within 2 to 4 weeks, with more significant results building as the dose increases over the following months.
What happens if I miss a dose of Mounjaro?
If you miss a dose and there are more than 4 days (96 hours) until your next scheduled dose, take the missed dose as soon as possible. If there are fewer than 4 days until your next dose, skip the missed one and continue with your regular schedule. Never take two doses at the same time.
Can I take Mounjaro every other week?
Mounjaro is designed to be taken once weekly for best results. Taking it every other week is not recommended, as it prevents the medication from reaching and maintaining the steady levels needed for effective appetite suppression and weight loss. Speak to your clinician before making any changes to your dosing schedule.
How often should I weigh myself on Mounjaro?
We recommend weighing yourself once every one to two weeks, at the same time of day and under the same conditions. Daily weigh-ins can be misleading and mentally exhausting, as your weight naturally fluctuates due to water retention, hormones, and digestion.
Will I gain the weight back when I stop taking Mounjaro?
Research shows that some weight regain is common after stopping Mounjaro, particularly if healthy habits are not maintained. This is why building sustainable dietary and exercise habits during treatment is so important. The goal is to use Mounjaro as a catalyst for lasting lifestyle changes, not as a permanent solution.
Is Mounjaro safe?
Mounjaro (tirzepatide) has been approved by the MHRA for weight management in the UK. It was studied in large clinical trials involving over 5,000 participants. The most common side effects are gastrointestinal (nausea, constipation, diarrhoea) and are typically mild to moderate. Serious side effects are rare. Your clinician will assess your suitability before prescribing and monitor you throughout treatment.
How does Mounjaro compare to Wegovy?
Both Mounjaro and Wegovy are effective weight loss medications, but they work slightly differently. Mounjaro targets both GIP and GLP-1 receptors (dual action), while Wegovy targets only GLP-1 (single action). In comparative studies, tirzepatide (Mounjaro) produced greater average weight loss than semaglutide (Wegovy) over the same period. However, the best choice depends on your individual circumstances. We offer both treatments at Longridge Travel Health Clinic and can help you decide which is right for you.
